Heavy fog, speed and reduced visibility has been blamed for the death of a truck driver after his vehicle collided with a freight train.
The Australian Transport Safety Bureau found there was a poor amount of signs warning motorists of the level crossing, which was not fitted with boom gates or flashing lights.
The impact caused 41 of the trains 64 wagons to derail.It took almost a week for his body to be retrieved from the wreckage.
